Article 14 of Public School Code requires students entering school, in third grade and in seventh grade to have dental examinations by a dentist or school district hygienist. Below is the PA Private Dental Exam form for parents to use if they choose to have a private dentist conduct their child’s required dental examinations. The form should be completed by the child's dentist and turned in at their child’s school.
